Let's take the OP's process as an example. The trigger is that a new form response has been submitted, and the action is to record that response in Excel.
What you think is happening is that Forms tells Power Automate "Hey here is a new form response, you wanted to be notified, now do your stuff". This is usually called event based processing.
What is actually happening is that Forms doesn't do any of that. In fact, none of the Power Automate connectors do any of that. Instead, Power Automate asks Forms "Hey, can I have your list of forms that have been added since the last time I asked?". "Hey, can you give that to me again?". "Sorry I need it again" etc. This is usually called polling. For a variety of reasons (Shannon/Nyquist, clock differences, network issues etc) the result of the polling may be misleading. Let's say the polling runs every minute. A form is created and then immediately modified. The next polling would ask for newly created forms and would miss this one since it is no longer "new".
With polling there is no 100% guarantee that all "events" are captured correctly as per the trigger definition. The OP noticed because the sequence is out of whack. Now you could add a process part that fetches the missing forms entries, but that is post fact. No way to anticipate.

I don't have a remedy but I want to remind you that all of these "triggers" in Power Automate are fake. Everything is done through polling, and it is possible that the polling process misses a few changes every now and then. Plan your processes accordingly.

Just this week the Microsoft Form is skipping recording some submissions in the Excel file in the background. As you can see entries 113 & 114 do not appear. I know I can find them if I go to the Microsoft Form and manually add in the detail but I am not sure why this is occuring. Has anyone experienced this? I have checked and this spreadsheet is open in the online (not app version) of Excel.
